Please bring the following to your exam: A valid photo ID Your vision insurance card (if applicable) Your current glasses or contact lenses A list of any medications you are currently taking, as some can affect eye health Any previous prescription records, if available If you are a new patient, plan to arrive a few minutes early to complete your intake forms.
